A flush floor vent is a two-piece unit: a frame set into the floor with its top even with the finished surface, and a slatted insert that drops into the frame. In hardwood the frame is the same species, unfinished or prefinished. In LVP and laminate it is a matched vinyl unit or a wood frame stained to the plank color. A drop-in register, by contrast, sits on top of the floor and is held there only by its own weight.
Flush vents go in easiest while the floor is being laid, because the boards are cut to the frame. They can be retrofitted into an existing floor too: the opening is squared to the frame size, a rabbet is cut for the frame lip, and the frame is set on the subfloor and fastened. The duct boot underneath is usually the problem. It sits at the old floor height and has to be trimmed down or shimmed up so the insert does not rest on it. We do that adjustment; a duct that has to move goes to an HVAC contractor.

Situations and options
Flush vents in new hardwood
Frames set as the floor goes down, boards cut to the frame, sanded and finished with the floor when the floor is site-finished. The cleanest result.
Retrofit into existing hardwood
The old drop-in register comes out, the hole is squared and rabbeted for the frame, the frame is set flush and the insert dropped in. Unfinished frames are stained to match on site.
Flush vents in LVP or laminate
A matched vinyl vent or a wood frame in a floating floor. The frame is fastened to the subfloor and the floor keeps its expansion gap around it, so the floor can still move.
Cold air return grilles
Bigger openings in the floor or at the base of a wall. Flush wood return grilles take a stronger frame and more fastening. Wall returns get a wood grille with a frame that matches the base.

Can flush floor vents be installed in an existing hardwood floor?
Yes. The old register comes out, the opening is squared to the frame size, a shallow rabbet is cut around it for the frame lip, and the frame is set on the subfloor and fastened. An unfinished frame is stained to match on site, or a prefinished one in the same species is used. The seam shows a little more than in a new floor, but far less than a metal register.
Do flush vents work in LVP or laminate?
Yes, with two things done right. The frame is fastened to the subfloor, not to the floating floor, and the floor keeps its expansion gap around the frame so it can move with the seasons. Matched vinyl vents are made for most plank lines; a wood frame stained to the plank color works too.
What size floor vent do I need?
Measure the duct opening in the floor with the old register pulled out, not the register itself, because registers are labeled by the hole they fit. Supply boots come in a few standard sizes; returns are bigger and vary more. Send a tape-measure photo and we confirm the size before ordering.
Will a wood vent restrict airflow?
A wood insert has thicker slats than a stamped metal one, so the open area is a little smaller. On a normal supply run the difference is not noticeable. On a return, which moves more air, we use a grille with wider slat spacing or size up the frame. A duct boot sitting too high under a flush vent blocks more air than the slats do, which is why we trim or reset the boot.
Can you replace all the registers in my house at once?
Yes, and it is the best way to price it. Count the supply vents and the returns, send one photo of a typical boot, and the quote covers the set. A main floor of vents is usually one day. Bedrooms with carpet keep their drop-in registers; only the hard-floor rooms get flush ones.
Do you move or resize the ductwork?
We trim or raise the duct boot so the flush frame sits right, and reseal it to the subfloor. Moving a duct, adding a run or changing a boot size is HVAC work, and we bring in an HVAC contractor for it.
