  • Certificate of insurance on request
  • W-9 on request
  • Additional-insured wording asked of our agent
Green-sided home front with a new bay window and entry

Property managers and owners of apartment buildings rarely let a contractor start without a vendor file. The usual pieces: a certificate of insurance, a W-9 for the 1099 at year end, a license number, and sometimes the owner or management company named as additional insured on the certificate. Some companies use an online vendor portal; others send a packet.

We send the certificate of insurance and a W-9 on request. When your requirements call for specific wording, such as additional insured, a certificate holder name and address, or a waiver of subrogation, send us the exact requirement. We pass it to our insurance agent, who issues the certificate. What the agent can add depends on our policy, so we tell you up front if a request cannot be met rather than after the bid.

The license number on our paperwork is Minnesota residential building contractor BC809005. Minnesota has no state license for work on buildings of five or more units, so for those buildings we check the city's contractor licensing and permit rules before we bid and handle any city license or permit the job needs.

  • City permit and any city contractor license the building needs
  • Fire-rated corridor doors, where they apply
  • Exterior access on upper floors
  • Phasing: one mobilization or several

Send your vendor requirements with the bid request so the paperwork is done before you award the work.

Apartment Contractor Certificate of Insurance and Vendor Setup: questions, answered

Can I get a certificate of insurance from my window contractor?

Yes. JMB Builders sends a certificate of insurance on request. Send the certificate holder name and address and any wording your company requires.

Will you name our company as additional insured?

We ask our insurance agent to issue the certificate with your additional-insured wording. Whether the agent can add it depends on our policy. If it cannot be done, we tell you before the bid is awarded.

Do you provide a W-9 for apartment work?

Yes, on request, for your vendor file and the 1099 at year end. We are paid by check or direct deposit.

What license number do you use for apartment buildings?

Our Minnesota residential building contractor license is BC809005. Minnesota has no state license for buildings of five or more units, so we check the city's contractor licensing and permit rules for that building before we bid.

Can you fill in our vendor compliance portal?

Yes. Send the portal link or the packet with the bid request. We fill it in and upload the certificate and W-9 so the file is complete before the work is awarded.

What insurance limits do you carry?

The certificate shows the current coverage and limits. Send your requirements and we will tell you whether we meet them before you spend time on the bid.
